RESSECÇÃO CORNUAL UTERINA E SALPINGECTOMIA DIREITA LAPAROTÔMICA SEGUIDA DE CURETAGEM UTERINA VIA VAGINAL POR GESTAÇÃO HETEROTÓPICA: UM RELATO DE CASO
Gravidez heterotópica é a
coexistência de gestação ectópica e tópica,
cuja incidência tem apresentado aumento
nas últimas décadas (atribuído às técnicas de
reprodução assistida), variando de 1:3.889 a
1:6.778. Gravidez ectópica cornual ou intersticial
é caracterizada pela implantação do concepto na
junção da tuba com o corpo uterino. Representa
2 a 4% do total das gestações ectópicas e está
associada a 2 a 3% da taxa de mortalidade
materna. Objetivando apresentar uma patologia
rara, com possíveis repercussões maternas de
emergência e, por tanto, de importância médica
na detecção precoce e resolução assertiva.
O caso relatado demonstrou a importância da
ultrassonografia no diagnóstico, por assim ser o
considerado exame padrão ouro para detecção
precoce da patologia para todas as pacientes
com essa suspeita, tendo uma sensibilidade
de 87% e especificidade de 99% para
gestações ectópicas. A gestação intersticial
apresenta risco aumentado para complicações
hemorrágicas, visto sua proximidade anatômica
aos vasos uterinos e consequentemente
maior mortalidade associada. Sua incidência
vem crescendo nas últimas décadas, pois os
fatores de risco para essa patologia também
aumentaram. O caso exemplifica uma patologia
de importância médica, pois a detecção precoce
altera significativamente o prognóstico para
essas pacientes.

Abstract:
Heterotopic prenancy is the
coexistence of ectopic and topic gestation,
whose incidence has been increased along
decades (attributed to the assistes reproduction
techniques), varying from 1: 3,889 to 1: 6,778.
Cornual or interstitial ectopic pregnancy is
characterized by the implantation of the concept
at the junction of the tuba with the uterine body. It accounts for 2 to 4% of all ectopic pregnancies and is associated with 2 to 3% of
the maternal mortality rate. Aiming to present a rare pathology, with possible maternal
repercussions of emergency and, therefore, of medical importance in the early
detection and assertive resolution. The reported case demonstrated the importance of
ultrasonography in the diagnosis, thus being considered the gold standard examination
for early detection of the pathology for all patients with this suspicion, having a sensitivity
of 87% and specificity of 99% for ectopic pregnancies. Interstitial gestation presents
an increased risk for hemorrhagic complications, due to its anatomic proximity to the
uterine vessels and consequently higher associated mortality and its incidence has
been increasing in the last decades, since the risk factors for this pathology have
also increased. The case exemplifies a pathology of medical importance, since early
detection significantly alters the prognosis.
