RELEVÂNCIA DA ATUAÇÃO DO CIRURGIÃO-DENTISTA EM ÂMBITO HOSPITALAR: UMA REVISÃO DE LITERATURA
A higiene bucal de pacientes intubados, acamados e em Unidades de Terapia Intensiva (UTIs) torna-se deficiente pela impossibilidade do autocuidado, acarretando no acúmulo do biofilme dental e o desequilíbrio da microbiota oral residente pela colonização de microrganismos patogênicos. Existe uma relação importante entre o aparecimento e/ou o agravamento de distúrbios sistêmicos e o estado de saúde bucal dos pacientes hospitalizados por longos períodos de internação. Objetivos: Analisar e evidenciar a importância do Cirurgião-Dentista na manutenção da saúde bucal de pacientes hospitalizados. Métodos: Foi realizada uma revisão de literatura na base de dados BVS Odontologia, PubMED e SciELO, com os seguintes descritores: Unidade Hospitalar de Odontologia (Dental Service, Hospital), Saúde Bucal (Oral Health) e Hospitalização (Hospitalization). Foram selecionados 20 artigos nos idiomas inglês e português, nos anos 2003 a 2019. Houve descarte de artigos que analisaram situações fora área de atuação do Cirurgião-Dentista ou competentes à área de Cirurgia Buco-maxilo-facial. Resultados: De acordo com a análise da literatura, observou-se que a prevenção e a recuperação da saúde bucal de pacientes debilitados em âmbito hospitalar reduzem fatores que podem influenciar negativamente no seu estado de saúde geral, levando a diminuição da morbidade e mortalidade, melhora na qualidade de vida e diminuição no tempo de internação dos mesmos. A redução dos custos hospitalares e a rotatividade dos leitos são indicadores do trabalho da Odontologia e dos profissionais que assistem o paciente, uma vez que impedem que casos de baixa complexidade evoluam para casos mais graves. Conclusão: De acordo com a limitação de artigos pesquisados, pode-se concluir que a manutenção da saúde bucal é um aspecto a ser considerado para a condição sistêmica do paciente, e que o atendimento multidisciplinar com a incorporação do Cirurgião-Dentista nas equipes hospitalares contribui para a implementação de protocolos específicos para saúde bucal e assistência integral do paciente.

Abstract:
: Introduction: The oral hygiene of intubated patients, bedridden patients and the ones in the Intensive Care Unit (ICU) becames defective due to the inability of self-care, resulting in dental plaque build-up and the imbalance of the resident oral microbiota on account of the colonization by pathogenic microorganisms. There is an important correlation between the emergence and/or worsening of systemic disorders and the oral health conditions of patients hospitalized for long periods of hospitalization. Objective: This research study aimed to evaluate and emphasize the importance of dentistry in hospitals and the need for dental treatment in hospitalized patients. Methods: It is a literature review carried out at the BVS Odontologia, PubMED and SciELO databases, considering the keywords Dental Service, Hospital, Oral Health and Hospitalization. Twenty articles were selected in English and Portuguese, between 2003 and 2019. The articles that doesn’t include the dentist's practice or Oral and Maxillofacial Surgery area were disregarded. Results: According to the literature review, it was observed that the prevention and recovery of the oral health of debilitated patients in the hospital reduces factors that can negatively influence their general health conditions, decrease the morbidity and mortality, improving quality of life and reducing the hospital stay. The reduction in hospital costs and the turnover of beds are indicators of the work of dentistry and the professionals who assist the patient, once they prevent that simple cases gets worst. Conclusion: According to the limitation of researched articles, it can be concluded that maintaining of oral health is an aspect to be considered for the systemic condition of the patient, and the inclusion of the dental surgeon in the hospital multidisciplinary care contributes to the implementation of specific protocols for oral health and integral attention to the patient.
