RELATO DE EXPERIÊNCIA SOBRE PRÁTICAS GERENCIAIS, EDUCATIVAS E ASSISTENCIAIS DO MÉDICO EM UM BANCO DE LEITE HUMANO
A Rede Brasileira de Bancos de Leite Humano (BLH) é considerada a maior e mais complexa do mundo pela Organização Mundial da Saúde. Dada a importância do leite humano, o médico responsável técnico assume papel assistencial, de gerenciamento da unidade e dos profissionais, orientação, promoção e incentivo às doações. Durante a Pandemia de COVID-19, criaram-se estratégias que garantissem a manutenção da doação de leite humano e o atendimento à nutrizes com dificuldade em amamentar, proporcionando segurança a todos os envolvidos no processo, da doação à oferta. Objetiva-se descrever as ações direcionadas pela coordenação de um BLH em um hospital de referência para COVID-19. Trata-se de um estudo descritivo, tipo relato de experiência da vivência da Coordenação Médica do BLH sobre as estratégias adotadas frente à pandemia de COVID-19. Para garantir segurança, o BLH desenvolveu estratégias alinhadas a protocolos oficiais. Tendo mudado o formato assistencial, foi traçado um plano de ação pela equipe multidisciplinar da instituição, instituindo treinamento de novas práticas de biossegurança, limpeza terminal, distanciamento interpessoal mínimo e fluxo de transporte e manipulação do Leite Humano. Evitando aglomerações, o serviço passou a ser triado, com poucos atendimentos presenciais, serviço de orientação por telefone ou drive-thru, com o intuito de informar sobre a coleta, armazenamento e fornecer materiais de coleta em casa. O atendimento às nutrizes em UTI, antes realizado prioritariamente pelo BLH, passou a ser exercido por profissionais do setor, evitando contaminação cruzada. A equipe de enfermagem foi capacitada para identificar e atuar em situações como, ingurgitamento mamário, mastite, estímulo à lactação. Situado em um hospital referência em COVID-19, o BLH teve sua rotina readaptada frente ao alto risco de contaminação. Para isso, a ação conjunta de profissionais foi fundamental para assegurar que a coleta e distribuição de leite humano ocorresse em segurança, evitando complicações resultantes da interrupção da assistência.

Abstract:
The Brazilian Network of Human Milk Banks (BLH) is considered the largest and most complex in the world by the World Health Organization. Given the importance of human milk, the medical technician in charge assumes a role of assistance, management of the unit and professionals, guidance, promotion and encouragement of donations. During the COVID-19 Pandemic, strategies were created to ensure the maintenance of human milk donation and care for nursing mothers with difficulty breastfeeding, providing security to all those involved in the process, from donation to offering. The objective is to describe the actions directed by the coordination of a HMB in a referral hospital for COVID-19. This is a descriptive study, type experience report of the experience of the Medical Coordination of the HMB on the strategies adopted in the face of the COVID-19 pandemic. To ensure safety, the HMB developed strategies aligned with official protocols. Having changed the care format, an action plan was drawn up by the institution's multidisciplinary team, instituting training in new biosafety practices, terminal cleaning, minimum interpersonal distance and flow of transport and handling of Human Milk. Avoiding agglomerations, the service started to be screened, with few face-to-face calls, telephone or drive-thru guidance service, with the aim of informing about collection, storage and providing collection materials at home. The care of nursing mothers in the ICU, previously carried out primarily by the HMB, is now performed by professionals in the sector, avoiding cross-contamination. The nursing team was trained to identify and act in situations such as breast engorgement, mastitis, stimulation of lactation. Located in a reference hospital for COVID-19, the HMB had its routine readjusted in view of the high risk of contamination. For this, the joint action of professionals was essential to ensure that the collection and distribution of human milk took place safely, avoiding complications resulting from the interruption of care.
