RELATO DE CASO CLÍNICO: CERATOCISTO ODONTOGÊNICO EM MANDÍBULA TRATADO POR MEIO DE DESCOMPRESSÃO SEGUIDA DE ENUCLEAÇÃO COM OSTECTOMIA PERIFÉRICA
O ceratocisto odontogênico (CO) é uma lesão benigna, classificada como
cisto odontogênico de crescimento lento e assintomático, a prevalência é maior em
pacientes do sexo masculino, na região dos molares inferiores e ramo mandibular.
Com alta taxa de recidiva, os tratamentos variam de conservadores a agressivos,
dependendo das características da lesão. Objetivo: relatar o caso de um paciente
do sexo masculino, de 70 anos, que apresentou um CO na região de ramo e ângulo
mandibular direito, tratado por meio de descompressão e seguida de enucleação com
ostectomia periférica. Método: análise das informações obtidas a partir do prontuário
do paciente e do registro fotográfico dos métodos de diagnóstico e tratamento.
Resultados: baseado em biopsia incisional, foi realizada a descompressão da lesão a
fim de que ocorresse a neoformação óssea para proteção do feixe alveolar inferior e
diminuição dos riscos de fratura durante a remoção do cisto. O paciente foi orientado
a higienizar o interior da lesão, irrigando a cavidade que foi aberta com solução
fisiológica (0,9%) três vezes ao dia. Após nove meses de acompanhamento, constatouse
a formação óssea esperada. Assim, seguiu-se o tratamento com a enucleação e
ostectomia periférica. Aproximadamente dois anos após a remoção do CO, não foram
vistos sinais ou sintomas de recidiva. O paciente será acompanhado por mais três
anos para ser considerado curado. Conclusão: os bons resultados constatados na
literatura, somados ao obtido no presente caso, têm nos incentivado a tratar os CO de
forma conservadora por meio de enucleação com ostectomia periférica, associados ou
não à descompressão prévia.

Abstract:
The odontogenic keratocyst (OKC) is a benign lesion, classified as a slow
growth and asymptomatic odontogenic cyst with greater prevalence located in the region
of lower molars and mandibular ramus of men. With a high rate of recurrence, their
treatments range from conservative to aggressive, depending on the characteristics of
the lesion. Objective: To report a case of a male patient, 70 years old, who presented a
OKC affecting the ramus and the right mandibular angle, successfully treated by means
of decompression followed by enucleation with peripheral ostectomy. Method: analysis
of the information obtained from the patient’s medical records and photographic record
of the methods of diagnosis and treatment. Results: Based on an incisional biopsy,
a decompression of the lesion was performed in the bone neoformation protect the
inferior alveolar beam and decrease the risk of fracture during the removal of the cyst.
The patient was advised to sanitize the interior of the lesion, irrigating the cavity that
was opened with saline solution (0.9%) three times a day. After nine months of followup,
we found the expected bone formation. Thus, it was followed by treatment with
enucleation and peripheral ostectomy. Approximately two years after the removal of
OKC, were not seen signs or symptoms of recurrence. The patient will be accompanied
by three more years to be considered cured. Conclusion: The results found in the
literature, added to that obtained in the present case, have encouraged us to treat the
OKC conservatively by enucleation with peripheral ostectomy, associated or not with
prior decompression.
