RELAÇÃO ENTRE CIRURGIA BARIÁTRICA E FERTILIDADE FEMININA: UMA REVISÃO SISTEMÁTICA
A obesidade tem etiologia complexa e multifatorial, associando-se, muitas vezes, a problemas ginecológicos, como infertilidade feminina. A cirurgia bariátrica (CB) é a terapia indicada para obesos grau III (IMC≥40kg/m2) ou II (IMC≥35kg/m2) com comorbidades. O objetivo das autoras foi esclarecer a relação entre CB e fertilidade feminina e seus efeitos na reprodução e na Síndrome do Ovário Policístico (SOP). Trata-se de revisão sistemática da literatura realizada por meio da busca de artigos dos últimos 10 anos nas bases PubMed e SCOPUS. Os estudos incluíam, obrigatoriamente, mulheres em idade fértil, com ou sem SOP, submetidas à CB. A seleção de estudos e extração de dados foram realizadas em duplicata. Cinco dos artigos incluídos estudaram efeitos pós-bariátrica nos níveis de hormônio antimulleriano (HAM), comparando mulheres obesas com e sem SOP. Neles, a CB foi responsável por importante redução dos níveis de HAM (p<0,005). Em mulheres com SOP, essa redução levou à normalização dos níveis hormonais. Dois artigos avaliaram os efeitos de outros hormônios e mulheres com infertilidade pré-diagnosticada. Houve melhora na regularidade menstrual, na indução da ovulação, no déficit de excreção urinária de pregnanediol glicuronídeo lúteo (Pdg) e aumento do LH após cirurgia, melhorando a fecundidade. Os estudos demonstram significativa redução dos níveis de HAM após CB, porém insuficiente para determinar infertilidade. Alguns resultados são encorajadores quanto à indicação da CB como terapêutica para obesas inférteis que desejam engravidar. Porém, são necessários estudos que analisem a correlação da bariátrica com alterações dos níveis de HAM e sua importância clínica em relação à fertilidade.

Abstract:
Obesity has a complex and multifactorial etiology, often associated with gynecological problems, such as female infertility. The therapy recommended for obese grade III (BMI≥40kg / m2) or II (BMI≥35kg / m2) comorbidities is Bariatric surgery (BS). The authors' objective was to make the relationship between BS and female fertility and its effects on reproduction and Polycystic Ovary Syndrome (PCOS). This is a systematic review of the literature carried out by searching for articles from the last 10 years in PubMed and SCOPUS databases. The studies mandatorily included women of childbearing age, with or without PCOS, submitted to BC. The selection of studies and data extraction were carried out in duplicate. Five of the included articles studied the post-bariatric effects on the levels of antimullerian hormone (AMH), comparing obese women with and without PCOS. In those, BS was responsible for an important reduction in AMH levels (p <0.005). In women with PCOS, this reduction led to the normalization of hormone levels. Two articles evaluated the other effects of the hormones and women with pre-diagnosed infertility. There was an improvement in menstrual regularity, in the induction of ovulation, without a deficit of urinary pregnanediol luteal glucuronide (PdG) and an increase in Luteinizing Hormone (LH) after surgery, improving fertility. Studies have shown a reduction in AMH levels after BS, but insufficient to determine infertility. Some results are encouraging regarding the indication of BS as a therapy for infertile obese women who wish to become pregnant. However, studies that analyze the correlation of bariatrics with changes in AMH levels and its clinical importance in relation to fertility needs to be improved and new studies should be developed in order to make this relationship clear.
