REFLEXÕES SOBRE A RELAÇÃO MÉDICO-PACIENTE NO TRATAMENTO ONCOLÓGICO, UMA PERSPECTIVA MÉDICA
A relação médico e paciente é o alicerce da clínica, principalmente na oncologia, em que o cuidado e o vínculo devem ser bem estabelecidos para obter sucesso na adesão ao tratamento, bem como resiliência no enfrentamento da doença, promovendo uma saúde mais humanizada. Este trabalho propôs avaliar o perfil da comunicação entre o médico e o paciente oncológico, considerando o impacto emocional de ambos frente aos fatores relacionados a fisiopatologia da doença e os seus impactos para o paciente, na perspectiva médica. Trata-se de um estudo transversal, descritivo e qualitativo, no qual um grupo amostral de médicos associados a UBS’s municipais foram submetidos a um questionário, composto por 11 perguntas objetivas e 01 discursiva. Os dados obtidos foram confrontados com publicações da literatura dos últimos 20 anos. Dentro do grupo amostral, 26 médicos aceitaram participar da pesquisa. Segundo esses, a relação entre o médico e o paciente é de extrema importância, porém, como o profissional não recebe formação para tal, deve adquiri-la através da prática. As principais dificuldades apontadas foram: pouco tempo para muita demanda na rede pública; dificuldade em iniciar certos assuntos e estabelecer uma conversa e falta de empatia. O trabalho na oncologia é árduo, principalmente no que se refere a transmitir notícias ruins aos pacientes e seus familiares, porém, faz-se necessário e foi observado prevalência de profissionais que conseguem separar o impacto desses da vida pessoal. Frequentemente os pacientes recebem informações sobre o enfrentamento da doença e, quando em estado terminal, a família evita que o paciente conheça a verdade, a fim de minimizar a angústia e o sofrimento com a doença. Apenas um participante ainda não se sente confortável em abordar a espiritualidade com seus pacientes, o que reforça a importância do coping religioso-espiritual para os processos de saúde e doença, principalmente na oncologia.

Abstract:
The doctor-patient relationship is the foundation of the clinic, especially in oncology, in which care and bonding must be well established for successful adherence to treatment, as well as resilience in coping with the disease, promoting more humanized health. This work proposed to evaluate the communication profile between the doctor and the cancer patient, considering the emotional impact of both in the face of the factors related to the pathophysiology of the disease and their impact on the patient, according doctor opinion. This is a cross-sectional, descriptive and qualitative study, in which a sample group of doctors associated with municipal UBS’s were submitted to a questionnaire, consisting of 11 objective questions and 01 discursive questions. The results obtained were compared with literature publications from the last 20 years. Among the invited doctors, 26 agreed to participate in the research. According to them, the relationship between the doctor and the patient is extremely important, however, as the professional does not receive training for this, he must acquire it through practice. The main difficulties pointed out were: little time for a lot of demand in the public network; difficulty in starting certain subjects and establishing a conversation and lack of empathy. The work in oncology is hard, especially when it comes to transmitting bad news to patients and their families, however, it is necessary and there was a prevalence of professionals who can separate their impact on their personal lives. Often patients receive information about coping with the disease and, when in a terminal state, the family prevents the patient from knowing the truth, in order to minimize anguish and suffering with the disease. Only 1 doctor still does not feel comfortable approaching spirituality with his patients, which reinforces the importance of religious-spiritual coping for health and disease processes, especially in oncology.
