Abstract: Introduction: Therapeutic bond is defined as the mutual construction of a relationship based on respect, understanding, and support throughout the therapeutic process. Despite its importance, information on how occupational therapists establish this alliance is limited. The aim of this study is to understand how occupational therapists and children and adolescents in social protection homes in Chile shape their connections. Method: A qualitative study was conducted through semi-structured interviews with three occupational therapists working in child protection residences in different regions of Chile, who voluntarily participated through informed consent. Content analysis was performed using a procedure that encompassed preanalysis, definition of units of analysis, establishment of rules and classification codes, development of categories and subcategories of analysis, and the final integration of findings. Results: Categories and subcategories of analysis related to the occupational therapist's academic background, therapeutic alliance, role in social protection homes, characteristics of therapeutic bonding, and epistemological positioning, among other aspects, were identified. The main results indicate that therapeutic bonding allows children and adolescents to trust the occupational therapist, thus favoring the intervention process. Key factors in establishing these bonds include soft skills, previous bonding experiences, and ethical judgment on the part of professionals. Conclusion: From a socio-critical and human rights perspective, the importance of therapeutic connection between occupational therapists and children and adolescents residing in social protection homes is underscored. This element stands as a fundamental pillar for cultivating effective therapeutic practices and enhancing the quality of life for this segment of the population in Chile. Emphasis is placed on the urgent need to forge an intentional and bidirectional therapeutic bond that, by optimizing the occupational performance of children and adolescents, promotes their holistic development. Ultimately, the significance of creating supportive environments that positively impact the overall wellbeing of children and adolescents is highlighted. This involves promoting friendly, empathetic, and respectful ways of establishing bonds in their living environments. In this context, the conscious construction of therapeutic relationships emerges as an essential pathway to shape a more promising present and future for those who, due to various circumstances, coexist in social protection homes.
