Abstract: Early Childhood Caries (ECC) is a rapidly progressing and severe chronic condition, primarily associated with a cariogenic diet and inadequate oral hygiene practices. Clinical manifestations include infection, pain, impaired mastication and phonation, behavioral changes, reduced academic performance, premature loss of primary dentition, as well as psychological trauma and diminished self-esteem. For the aesthetic management of primary teeth affected by extensive carious lesions, composite resins are commonly employed. The present study aims to report a clinical case of aesthetic and functional rehabilitation of anterior primary teeth utilizing composite resin through the direct freehand technique and the use of a celluloid (acetate) crown. The methodology was based on an observational, descriptive, and narrative study design, with data derived from clinical activities within the Pediatric Dentistry curriculum at a Public Higher Education Institution. The restorative procedures were performed using the direct freehand technique with composite resin on teeth 52, 61, and 62, which exhibited extensive coronal destruction. For tooth 51, which had previously undergone pulpectomy, restoration was achieved using a celluloid crown technique. The advantages of this approach include favorable durability, cost-effectiveness, restoration of both aesthetics and function, and reduced chair time. However, a notable limitation is that composite resin depends on effective adhesion to both enamel and dentin for optimal retention. In conclusion, the rehabilitation of anterior primary teeth with composite resin effectively restored oral function and aesthetics, contributing positively to the patient's quality of life and self-esteem, and playing a crucial role in the child’s overall development and social integration.
