Qualificação da Passagem de Plantão em Enfermagem por Meio de uma Ferramenta Estruturada: Relato de Experiência

• Abstract: Objective: to report the authors' experience during the construction, implementation, and review of a shift handover tool. Methods: This is a descriptive experience report with a qualitative approach based on the authors' experience during Curricular Extension Activity 8 (ACE 8), from July 7 to October 21, 2025. Among the activities carried out during ACE 8 are: identification/formulation of the problem; literature search/data collection; data evaluation; analysis of the findings of the articles included in the review, presentation and interpretation of the results, construction of a shift handover tool, implementation of the constructed tool, and review of the tool. Results: The evaluation of the shift handover tool involved six nurses from different institutions. Most had between one and five years of experience. The results showed good acceptance of the tool, with 100% of participants considering the information clear and adequate. The majority (83.3%) positively evaluated the organization of the fields and the time required to complete them. Some of the professionals suggested including new fields and adjusting the format of the instrument. In the overall evaluation, 66.7% rated the tool as “very good.” Conclusion: The experience allowed us to understand the importance of effective communication and standardization in shift handover for patient safety. The construction and application of technology favored the organization.
