QUALIDADE E SEGURANÇA NO PROCESSO MEDICAMENTOSO: RELATO DE EXPERIÊNCIA NA INVESTIGAÇÃO DE EVENTO ADVERSO
Falhas em qualquer uma das etapas que compõem a cadeia medicamentosa pode acarretar um incidente que poderia ser evitado. A notificação de um evento adverso associado ao uso de medicamentos serviu de alerta para a equipe do Núcleo de Segurança do Paciente (NSP) iniciar processo de investigação. O objetivo do estudo foi descrever as estratégias utilizadas pela equipe do NSP na identificação dos fatores contribuintes para ocorrência de evento adverso com uso do medicamento. Para tal, segue relato de experiência da investigação do evento em que um neonato recebeu equivocadamente um medicamento barbitúrico que estava prescrito para o paciente do leito ao lado. A investigação foi conduzida pela equipe do NSP seguindo o Protocolo de Londres. O resultado da investigação demonstrou a ocorrência de não conformidades, identificou-se as relevantes relacionadas ao ambiente (interrupções, barulho), a tarefa (quebra de protocolo), ao sistema (restrições financeiras, cultura e estrutura organizacional), a equipe (comunicação ineficaz, fatores cognitivos e emocionais, desempenho e excesso de confiança), ao paciente (complexidade e gravidade, dificuldade de compreensão da genitora, fatores sociais e de personalidade). Identificar falhas relacionadas a medicamentos é de grande valor para a instituição de saúde, pacientes, familiares e profissionais, pois esse tipo de evento é frequentemente notificado pelos NSP dos serviços de saúde de todo o país, o que gera aumento de custos no tratamento, internação hospitalar prolongada, além de desconforto e impacto negativo na qualidade de vida dos pacientes. A equipe investigadora entendeu que o Protocolo de Londres foi ferramenta facilitadora, que direcionou a análise dos eventos adversos e propiciou a condução adequada para promoção da cultura justa.

Abstract:
Flaws in any part of the medicine chains’ processes may result in evitable accidents. The notification of an adverse event associated with the use of medicines was a warming to the team of the Patient Safety Nucleus (PSN) started processes of investigation. The mainly reason of the study was describe the strategies used by PSN on the identification of contributors factors to the occurrence of adverse event with the use of drugs. With this objective, is reported the experience of event’s investigation which a newborn received mistakenly barbiturate drug prescribed to the patient in next. The inquirer’s result was realized by the PSN team following the London Protocol. The result exposed the occurrence of non-conformities, identified related relevant as the environment (interruptions, noise), the work (non-following of the rules), the system (financials difficult, culture and organization, the team (ineffective communication, cognitive factors and confident excess), the patient (complexity and gravity, geriatric understanding problems, social and personality aspects). Identify mistakes related with drugs is very important to the health’s institution, patients, familiars and professionals, because this kind of occurrence is frequently related by PSN in all country, increasing the costs of treatment, prolonged hospital stay, in addition with uncomfortable and negative impacts on patient’s life quality. The team understated that London Protocol was a usable mean, which directed the analyze of events e provided the right conduction to the far culture promotion.
