Abstract: Melasma is an acquired pigmentary disorder of multifactorial etiology, characterized by symmetrical cutaneous hyperpigmentation. Effective clinical management of this condition remains a challenge, especially in recurrent cases. Pycnogenol®, a standardized extract from the bark of Pinus pinaster, has demonstrated antioxidant, anti-inflammatory, and depigmenting properties, and is being investigated as an adjuvant in the treatment of melasma. In this context, this narrative review aimed to analyze the available clinical evidence on the efficacy of oral Pycnogenol® in the management of melasma, with a focus on its antioxidant effects and its role in reducing skin hyperpigmentation. A search was conducted in the PubMed, LILACS, and SciELO databases, including clinical studies published between 2000 and 2024. The findings indicate significant clinical improvement in severity scores and quality of life, with few adverse effects reported. It is concluded that Pycnogenol® represents a promising alternative; however, further controlled studies are needed to support the development of clinical guidelines.
