Abstract: Early puberty can be defined as the development of secondary sexual characteristics before 8 years of age in girls and before 9 years of age in boys, which can be caused by hormonal disorders, genetic alterations, or exposure to chemicals. The objective of this study was to analyze the main etiologies, diagnosis, epidemiology, clinical evolution, and treatment of early puberty, aiming to improve clinical understanding of the pathology. This was a literature review that included articles in Portuguese and English published between 2014 and 2024 from UpToDate, SciELO, and BVS databases. Articles outside this period, not available online, or duplicates were excluded. The review reveals that there are several etiologies for early puberty, classified into central causes (early activation of the hypothalamic-pituitary-gonadal axis) or peripheral causes (autonomous production of sex steroids). Depending on the etiology, diagnostic methods are chosen, which include clinical factors, hormonal assays, and imaging studies such as wrist radiography to assess bone age. Furthermore, the clinical progression of the disease presents initially with breast development in girls, increased testicular volume in boys, accelerated growth velocity, skeletal maturation, and the appearance of other secondary sexual characteristics. Regarding treatment, the aim is to suppress gonadotropin (LH and FSH) production using long-acting GnRH agonists in clinically indicated patients.
