PROTOCOLO INFERIOR IMEDIATO: DO PLANEJAMENTO À CONCLUSÃO - RELATO DE CASO CLÍNICO
Introdução: A prótese de carga imediata oferece ao paciente conforto e função durante a cicatrização do implante comparado com uma prótese convencional, fazendo com que os implantes recebam carga mastigatória sem a necessidade de esperar pelo processo de osseointegração. Objetivo: Observar a progressão do processo pós-cirúrgico após 6 meses com a instalação da prótese protocolo inferior em resina acrílica sobre os implantes realizando carga de maneira provisória após a instalação dos implantes. Descrição do caso clínico: O presente estudo é um relato de caso clínico em que a paciente foi submetida a uma prótese de protocolo provisório imediato sob implantes recém-instalados. O tratamento proposto foi o de realizar uma prótese do tipo protocolo inferior imediato em resina acrílica mantendo a prótese total superior. Após adequado planejamento multidisciplinar, foi selecionada a prótese, com seleção de novos componentes e realizada a cirurgia. Foi feito um acompanhamento após 6 meses. Discussão: Foi observada satisfação estética e funcional, após 6 meses de acompanhamento. A seleção dos componentes protéticos foi fundamental para o resultado final, sendo justificado devido o planejamento protético antes da cirurgia ser um dos principais aspectos das reabilitações estéticas sobre implantes. A escolha por uma prótese de protocolo inferior foi baseada em modelos que indicassem tratamentos com compatibilidade oclusal com prótese total, excelente funcionalidade, baixo custo, de execução simples e que se encaixasse com qualquer sistema de implantes. Além disso, prévio a cirurgia, foi realizada uma raspagem supragengival, com objetivo de reduzir a placa bacteriana, pois o sucesso dos implantes está relacionado à estética das reabilitações. Conclusão: Concluímos que o todo planejamento reverso desde a anamnese associado com o planejamento cirúrgico e protético, obteve sucesso na reabilitação, melhorando tanto a estética, função mastigatória e paciente.

Abstract:
Introduction: The immediate loading prosthesis offers the patient comfort and function during the cicatrization of the implant compared to a conventional prosthesis, making the implants receive a masticatory load without the need to wait for the osseointegration process. Objective: To observe the progression of the post-surgical process after 6 months with the installation of the inferior protocol prosthesis in acrylic resin on the implants, performing a provisional load after the installation of the implants. Description of the clinical case: The present study is a report of a clinical case in which a patient was submitted to a prosthesis with an immediate provisional protocol under newly installed implants. The treatment was proposed to perform a inferior protocol prosthesis in acrylic resin while maintaining a superior total prosthesis. After adequate multidisciplinary planning, the prosthesis was selected, with the selection of new components and surgery was realized. Follow-up was done after 6 months. Discussion: Aesthetic and functional satisfaction was observed after 6 months of follow-up. The selection of prosthetic components was fundamental for the final result, being justified due to the prosthetic planning before surgery being one of the main aspects of aesthetic rehabilitation on implants. The choice for a inferior protocol prosthesis was based on models that indicated treatments with occlusal compatibility with total prosthesis, excellent functionality, low cost, simple execution and that adapt to any implant system. In addition, previous to surgery, a supragingival scraping was realized with the objective of reducing plaque, as the success of the implants is related to the aesthetics of the rehabilitation. Conclusion: We conclude that the all reverse planning since an annamnesis associated with surgical and prosthetic planning, achieved success in rehabilitation, improving both the aesthetics, masticatory function and the patient's self-esteem.
