Abstract: As the population ages, there is an increase in the number of elderly people affected by chronic non-communicable diseases who require the use of multiple prescription drugs to treat them. Polypharmacy, a condition that consists of the use of four or more medications, in the elderly can be harmful to health. The use of multiple medications in the treatment of chronic non-communicable diseases, such as type 2 diabetes mellitus (DM 2), can act as a barrier to adherence to treatment, because it complicates pharmacological regimens, favoring drug and adverse interactions, in addition to contributing to the occurrence of errors in medication use, iatrogenesis and poor quality of life. In this sense, deprescribing is a commonly used method, using a careful and individualized approach to minimize the concomitant use of multiple medications in the elderly and, consequently, reduce their associated harms. Therefore, to improve therapeutic adherence in elderly patients with DM 2, an appropriate medical prescription is necessary, based on updated scientific evidence and person-centered care. Thus, this intervention project is justified, since it aims to improve adherence to the proposed therapeutic regimens by reducing polypharmacy.
