PROJETO DE IMPLANTAÇÃO DO PROGRAMA DE SEGURANÇA DO PACIENTE EM UMA MATERNIDADE PÚBLICA DA CIDADE DE SÃO PAULO.
Introdução: A preocupação com Segurança do Paciente existe desde 460 a.C. com Hipócrates pronunciando “Primeiro, não causar dano”. Em 1999, publicado relatório “To Err is Human”reconhece-se que profissionais de saúde cometem erros. Em 2004, Aliança Mundial para Segurança do Paciente lançada pela Organização Mundial de Saúde uniu responsáveis mundiais pelas políticas de saúde pública em prol à segurança do paciente. No Brasil, em 2013, Ministério da Saúde lança Portaria 529 e Programa Nacional de Segurança do Paciente enquanto Agência Nacional de Vigilância Sanitária publica RDC 36 e determina criação do Núcleo de Segurança do Paciente (NSP) nos serviços de saúde para executar as ações relacionadas a Segurança do Paciente. Objetivo: Implementar plano de segurança do paciente (PSP) em Hospital Público do Município de São Paulo. Método: Projeto de intervenção em hospital público gineco-obstétrico de grande porte, para implementação do PSP. O NSP foi instituído em 2014 com um profissional exclusivo. Estruturação da Comissão Multiprofissional de Gerenciamento de Risco e Segurança do Paciente com setores assistenciais, de apoio e alta direção. Iniciada disseminação da cultura da segurança do paciente, com sensibilização dos funcionários por meio de divulgações, treinamentos e participações em eventos e reuniões científicas. Elaborado formulário para sistema de notificação de incidentes. Resultado: Elaborado PSP por meio de brainstorming e baseado na ferramenta 5W2H para direcionar o plano de ação. Conclusão: Mesmo diante de dificuldades inerentes a instituição pública, obteve-se êxito nas atividades popostas: treinamentos e atividades de sensibilização visando às práticas seguras, melhoria em processos de trabalho e política de notificação de incidentes. Diante da tendência mundial com foco na segurança do paciente e da missão do hospital como Hospital de Ensino com Residências Médicas e Multiprofissional, destaca-se necessidade da inserção do tema na formação dos profissionais de saúde, ampliando conscientização e promovendo mudança efetiva da cultura de segurança.

Abstract:
Introduction: Concern for Patient Safety has existed since 460 BC with Hippocrates pronouncing “First, do no harm”. In 1999, a published report “To Err is Human” recognizes that health professionals make mistakes. In 2004, the World Alliance for Patient Safety launched by the World Health Organization brought together global policy makers for public health to promote patient safety. In Brazil, in 2013, the Ministry of Health launched Ordinance 529 and the National Patient Safety Program as the National Health Vigilance Agency publishes RDC 36 and determines the creation of the Patient Safety Nucleus (PSN) in health services to carry out actions related to Patient safety. Objective: Implement a patient safety plan (PSP) in a public hospital in the city of São Paulo. Method: Intervention project in a large public gynecological obstetric hospital, for the implementation of the PSP. PSN was established in 2014 with an exclusive professional. Structuring of the Multiprofessional Commission for Risk Management and Patient Safety with assistance, support and senior management sectors. Dissemination of the patient safety culture was initiated, with employee awareness through divulgation, training and participation in scientific events and meetings. Form for incident notification system developed. Result: Elaborated PSP through brainstorming and based on the 5W2H tool to direct the action plan. Conclusion: Even in the face of difficulties inherent to the public institution, there was success in the suggestion activities: training and awareness raising activities aiming at safe practices, improvement in work processes and incident notification policy. In view of the global trend with a focus on patient safety and the hospital's mission as a Teaching Hospital with Medical and Multiprofessional Residencies, the need to insert the theme in the training of health professionals is highlighted, increasing awareness and promoting an effective change in the safety culture.
