Abstract: Food allergies have become a growing public health problem, especially in childhood. This work aims to discuss how nutrition and the gut microbiota can contribute to the prevention of the development of food allergies in early childhood, considering maternal and infant nutritional habits in the first thousand days of a child's life. During this period, factors related to the mother's diet and infant nutrition directly influence the maturation of the immune system, the development of the gut microbiome, and the establishment of oral tolerance. Therefore, investigating how these factors relate to the risk of food intolerances and allergies in infants contributes to the advancement of scientific knowledge and to the basis of practices and public policies in maternal and child nutrition, aiming at reducing the incidence of these conditions and promoting adequate and healthy eating from the first years of life. This is a narrative review that brings together works published in the last 31 years in the Pubmed, Scopus, SciELO and Google Scholar databases. The results indicate that the use of infant formulas, unbalanced maternal diet and late introduction of allergenic foods can favor the development of food allergies. Conversely, exclusive breastfeeding, timely introduction of solid foods, and maternal consumption of unprocessed and minimally processed foods are associated with a healthier gut microbiota and strengthened immune tolerance. The first thousand days of life represent a window of opportunity for the formation of immunity and the prevention of food allergies. Thus, adequate nutritional strategies during pregnancy and complementary feeding are fundamental to promoting gut health and reducing the incidence of allergic diseases in childhood.
