Abstract: This study analyzes public health policies aimed at quilombola communities in Brazil, highlighting institutional advances, structural challenges, and the importance of social participation in ensuring the right to health. It is a qualitative, descriptive-analytical research based on bibliographic review and documentary analysis of legislation, governmental programs, and normative frameworks related to the topic. The discussion is grounded in the understanding of public policies as instruments for the implementation of social rights, situating health actions directed at quilombola populations within the broader context of historically constructed racial and social inequalities in Brazil. The findings indicate that, since the 2000s, significant institutional milestones have been established, including the creation of the Special Secretariat for the Promotion of Racial Equality Policies, the National Policy for the Promotion of Racial Equality, and the Brazil Quilombola Program. However, challenges remain, such as institutional bureaucracy, insufficient funding, inadequate infrastructure, and barriers to accessing primary health care services. The study concludes that promoting health equity for quilombola communities requires strengthened intersectoral public policies, sustained investments, and expanded social participation as mechanisms for democratization and public accountability.
