O envelhecimento da população e o

aumento das doenças crônicas demandam um

cuidado especifico dentro da atenção primária

a saúde, com isso a experiência vivenciada

tem o objetivo de Qualificar o cuidado domiciliar

de doentes crônicos e em cuidados paliativos;

Realizar o cuidado centrado no paciente

e Família; Melhorar o Acesso ao Cuidado;

Envolver a equipe multidisciplinar de saúde e a

família do paciente na continuidade do cuidado;

Otimizar o trabalho da equipe de Estratégia

Saúde da Família. Tendo como metodologia a

elaboração de planilha visando à classificação

dos pacientes de acordo com vulnerabilidade

apresentada nas visitas domiciliares no território

de atuação da Estratégia Saúde da Família

e equipe multidisciplinar de saúde. Após a

implementação da planilha de visitas, foram

realizadas 157 visitas domiciliares, pela mesma

equipe, demonstrando um aumento aproximado

de 155,4% no número de visitas domiciliares,

em relação ao segundo semestre de 2017, que

foram realizadas 61 visitas domiciliares.

Abstract:
The aging of the population and

the increase of the chronic diseases demand

a specific care within the primary attention to

health, with that the experienced experience

has the objective of Qualifying the home

care of chronic patients and in palliative care;

Perform care centered on the patient and

Family; Improve Access to Care; Involve the

multidisciplinary teamwork in health and the patient’s family in the continuity of care;

Optimize the work of the Family Health Strategy teamwork. Based on the methodology,

the elaboration of a spreadsheet aiming at the classification of patients according to

vulnerability presented in house calls in the territory of the Family Health Strategy

and multidisciplinary teamwork in health. After the implementation of the house calls

spreadsheet, 157 house calls were carried out by the same teamwork, demonstrating

an approximate increase of 155.4% in the number of house calls compared to the

second half of 2017, where 61 house calls were performed.
