Abstract: Pyoderma gangrenosum is a chronic, non-infectious, rare dermatosis that is difficult to diagnose and treat. It is manifested through ulcerated, painful skin lesions with rapid and progressive evolution, preferably in the lower limbs. In most cases, it is related to other pathologies, such as hematological diseases, arthritis, malignant neoplasms, inflammatory bowel disease, including Chron's disease, among others. In addition, exceptional cases associate cocaine use with the development of pyoderma gangrenosum. This study aims to report a case of pyoderma gangrenosum in a mixed-race, male, adult patient with Crohn's disease for 34 years and recently a former cocaine user, treated at the dermatology outpatient clinic. In addition to briefly addressing the pathophysiology, diagnosis, established treatment and the impact that this disease has on the patient's life.
