Abstract: This study investigated the profile and factors associated with smoking in patients with Chronic Obstructive Pulmonary Disease (COPD), based on evidence from 25 scientific articles. Findings show that active smoking is linked to worse clinical outcomes, higher exacerbation rates, accelerated functional decline, and reduced response to conventional therapies and pulmonary rehabilitation. Socioeconomic and cultural determinants were identified as major cessation barriers, while integrated and personalized strategies — including pharmacological, behavioral, digital technology, and complementary therapies — showed potential to optimize management. It is concluded that treating COPD smokers requires a multidimensional approach combining clinical actions and public health policies.
