PERFIL DE INTERVENÇÕES FARMACÊUTICAS EM PRESCRIÇÕES DE PROTOCOLOS ANTINEOPLÁSICOS E ACEITAÇÃO MÉDICA: UM TRABALHO MULTIPROFISSIONAL NA SEGURANÇA DO PACIENTE
Intervenção farmacêutica é o ato farmacêutico de prevenir, solucionar e manejar problemas relacionados à farmacoterapia. Em oncologia, a segurança do paciente é primordial para efetividade terapêutica, preservação da vida e de suas condições de saúde. Este trabalho é um relato da experiência de avaliação e validação da prescrição médica pela Farmácia Clínica de um Serviço Privado de Oncologia de João Pessoa - PB durante o período de 1 ano. Ao avaliar uma prescrição médica oncológica, o farmacêutico verifica os dados do paciente e as informações do seu protocolo terapêutico. Ao encontrar divergências, o documento é reencaminhado ao prescritor com a motivação, este examina a pertinência da intervenção, aceitando-a e corrigindo a prescrição ou recusando-a sob justificativa médica. No período do estudo, foram emitidas 10042 prescrições, das quais 100% foram avaliadas por farmacêuticos clínicos, gerando um total de 351 intervenções (3,5%) com taxa de aceitação de 99,4% (n = 349). As intervenções realizadas foram relacionadas a ajuste de dose 38,7% (n = 136) do total, inclusão de medicamento 22,5% (n = 79), adequação de diluente 10,5% (n = 37), substituição de medicamento 10,2% (n = 36), adequação de dia do ciclo 10,2% (n = 36) e suspensão de medicamento 8,8% (n = 31). As solicitações para ajuste de dose estavam relacionadas à redução por toxicidade do protocolo, as de substituição ou inclusão de medicamentos eram de pacientes com histórico de reação infusional que necessitam de preparo adicional ou substituição a um item prescrito sem afetar o protocolo oncológico. O nível de aceitação de intervenções farmacêuticas foi alto, isso alterou ou gerou novas prescrições mais seguras e corretas. Dessa forma, o trabalho multiprofissional contínuo contribuiu diretamente para a cultura de segurança do paciente.

Abstract:
Pharmaceutical intervention is the pharmaceutical act of preventing, solving and managing problems related to pharmacotherapy. In oncology, patient safety is essential for therapeutic effectiveness, life preservation and them health conditions. This work is an account of the experience of evaluating and validating of medical prescription by the Clinical Pharmacy of Private Oncology Service in João Pessoa - PB during a period of 1 year. For to evaluate the oncology prescription, the pharmacist checks the patient's data and information from their therapeutic protocol. When finding differences, the document is forwarded to the prescriber with the motivation, who examines the relevance of the intervention, accepting it and correcting the prescription or refusing it under medical justification. During the study period, 10042 prescriptions were issued, of which 100% were evaluated by clinical pharmacists, generating 351 interventions (3.5%) with an acceptance rate of 99.4% (n = 349). The interventions performed were related to dose adjustment 38.7% (n = 136) of the total, inclusion of medication 22.5% (n = 79), adequacy of diluent 10.5% (n = 37), drug replacement 10.2% (n = 36), cycle day adequacy 10.2% (n = 36) and drug suspension 8.8% (n = 31). Interventions for dose adjustment were related to the reduction by toxicity of the protocol, those for substitution or inclusion of medication were for patients with a history of infusional reaction who need additional preparation or replacement of a prescribed item without affecting the oncological protocol. The level of acceptance of pharmaceutical interventions was high, this changed or generated new prescriptions more safer and correct. Thus, the continuous multiprofessional work contributed directly to the patient safety culture.
