Abstract: This chapter discusses severe cerebral palsy (CP) as a neurological condition capable of modulating the intestinal environment, particularly in children with greater impairment. Based on a theoretical and conceptual approach, it analyzes the relationships among motor severity, Gross Motor Function Classification System (GMFCS) levels, feeding changes, gastrointestinal dysfunction and fecal composition. The text highlights that dysphagia, gastroesophageal reflux, constipation, dysmotility, reduced mobility, modified diets, epilepsy and frequent medication use may alter intestinal transit, fermentable substrate availability, luminal pH and microbiota metabolic activity. Evidence of intestinal dysbiosis in children with CP is also addressed, considering taxonomic and functional changes in the microbiome. Finally, the chapter supports the relevance of integrated fecal matrix analysis, including microbiota, microbial metabolism, biochemical markers and redox status, within the microbiota-gut-brain axis. This synthesis reinforces the need to interpret CP beyond an exclusively neuromotor perspective. It strengthens the biomedical rationale for future fecal investigations in this population.
