PAPEL DO FARMACÊUTICO NA CONCILIAÇÃO MEDICAMENTOSA DE MULHERES COM CÂNCER DE MAMA EM UM HOSPITAL ONCOLÓGICO
A atuação do farmacêutico hospitalar, nas últimas décadas, foi ampliada, somando atividades com foco na prática clínica e atenção farmacêutica. Entre as atividades em expansão, podemos citar a conciliação medicamentosa, um processo que envolve a elaboração de uma lista completa, precisa e atualizada dos medicamentos de uso contínuo do paciente, anterior à sua internação e a comparação desta lista com a prescrição médica. Pacientes com regimes medicamentosos oncológicos podem se beneficiar da conciliação medicamentosa, uma vez que estes medicamentos apresentam grande chance de causarem toxicidade. Este trabalho tem como objetivo mostrar a importância do profissional farmacêutico na conciliação medicamentosa de pacientes com câncer de mama atendidas em um hospital oncológico, no estado de Minas Gerais, Brasil. Trata-se de um estudo transversal observacional, onde foram avaliadas 34 fichas de conciliação medicamentosa de pacientes com câncer de mama atendidas no período de 02/01/2018 a 28/02/2018. Das prescrições analisadas, 66,7 % foram conciliadas, o que reforça a importância do profissional farmacêutico em promover o uso racional de medicamentos e possibilitar um melhor tratamento às pacientes.

The role of the hospital pharmacist has been expanded in recent decades by adding activities with a focus on clinical
practice and pharmaceutical care. Among the expanding activities, we can citethe medication reconciliation, a process that involves the elaboration of a complete, accurate, and updated list of the medications continuously used by the patient, prior to his hospitalization and the comparison of this list with the medical prescription. Patients with oncological drug regimens can benefit from drug reconciliation, since these drugs have a high chance of causing toxicity. This work aims to show the importance of the pharmaceutical professional in the medication reconciliation of patients with breast cancer treated at an oncology hospital, in Minas Gerais state, Brazil. An observational cross-sectional study was made with 34 medication reconciliation forms evaluated for patients with breast cancer treated from 01/02/2018 to 02/28/2018. Of the prescriptions analyzed, 66.7% were reconciled, which reinforces the importance of the pharmaceutical professional in the promotion of the rational use of medicines, enabling better treatment for the patients.
