Abstract: Background: Chronic liver disease (CLD) is a progressive deterioration of liver function, with cirrhosis as its final stage, associated with high mortality and morbidity in developing countries. People with advanced CLD experience frequent complications and hospitalizations. Preparation for discharge represents a transition that requires careful planning to ensure continuity of care, avoid readmissions, empower the patient and family, and prevent complications, thereby improving quality of life. Objectives: The objective of this Integrative Literature Review (ILR) is to gather knowledge about the role of nursing in the treatment of CHF, with a special focus on the process of preparing for hospital discharge. Method: The ILR was conducted according to Whittemore and Knafl (2005), and the research was carried out in the CINAHL and MEDLINE databases, guided by the question: “What nursing interventions (I) contribute to the preparation for discharge of people with CHF and their families (P), promoting a safe and effective transition to home (Co)?” in the context of nursing practice. Results: A total of 104 articles were obtained, of which 5 were selected. The articles identified that nursing practice in preparation for discharge should be a client-centered approach with CHF patients and their families, impacting their autonomy and quality of life, based on a clinical picture characterized by high complexity. Conclusion: DHC represents a significant challenge for nurses. This RIL reinforced the importance of nursing interventions in the hospital transition process, promoting therapeutic adherence, self-care, and family empowerment as pillars for improving prognosis and quality of life.
