Abstract: Testicular torsion is a urological emergency in which there is obstruction of blood flow to the testicles, which can lead to necrosis, gangrene and loss of the organ if it is not reversed within 6 hours of the onset of the condition. In view of this, this study will report the case of a young patient presenting with acute scrotum (testicular torsion) 15 hours after the onset of the condition with signs of ischemic orchitis, who was surgically treated and administered a solution containing Nipride (sodium nitroprusside), influencing the improvement of testicular perfusion, enabling the successful performance of orchidopexy and preservation of the organ. The information was obtained through review of medical records, photographic records, reports from the surgeon and resident physician present and literature review. The reported case brings to the discussion a therapeutic option for an emergency situation, which is Testicular Torsion, more than 6 hours after the onset of the condition, capable of obtaining a satisfactory and definitive result, without the need for Orchiectomy.
