Abstract: Minimum Intervention Dentistry (MID) proposes a paradigm shift in caries management, replacing G.V. Black's invasive surgical model with a medical-biological approach focused on tissue preservation. In Primary Health Care (PHC), clinical effectiveness depends on evidence-based protocols and the judicious selection of bioactive materials. This integrative review analyzed the efficacy of selective carious tissue removal and the clinical performance of Glass Ionomer Cement (GIC) compared to Composite Resin (CR), through a deep analysis of 20 high-impact scientific articles (2015-2025). In addition, the scientific data were correlated with the practical application of MID during the national mobilization "Oral Health Day B" in Gurupi-TO, where 652 procedures were performed on 250 children. The results show that selective removal is the gold standard for preserving pulp vitality, drastically reducing iatrogenic exposures. It is concluded that GIC, due to its bioactivity and remineralization capacity, is the most strategic material for PHC, a fact proven by the technical viability observed in the municipal action. The implementation of MID proves essential for the sustainability of the public health system and for the maintenance of the population's natural dentition.
