Abstract: Obesity has a strong relationship with Gastroesophageal Reflux Disease (GERD), worsening symptoms and complications. This article analyzes pathophysiological mechanisms and therapeutic strategies, comparing current evidence from the scientific literature. Abdominal obesity contributes to increased intra-abdominal pressure and dysfunction of the lower esophageal sphincter, facilitating reflux. Comparative analysis shows that gastric bypass is more effective in GERD control than sleeve gastrectomy, although the latter may be enhanced when combined with fundoplication techniques. Pharmacotherapy in obese patients also demands special attention, requiring dose adjustments. It is concluded that the ideal approach to GERD associated with obesity should be individualized, combining surgical intervention, pharmacological treatment, and lifestyle changes.
