O VÍNCULO TERAPÊUTICO EM UM CASO DE IDEAÇÃO SUICIDA: RELATO DE EXPERIÊNCIA
O presente trabalho refere-se ao
relato de um caso clínico atendido no Serviço
de Plantão Psicológico, em uma clínica-escola,
situada no interior do estado de São Paulo,
por uma estagiária do 5º ano do curso de
Psicologia, a uma paciente de 46 anos de idade.
O objetivo centra-se em ressaltar a importância
do estabelecimento do vínculo terapêutico no
atendimento de uma paciente com ideação
suicida. O Serviço possui como protocolo
previsto um atendimento inicial, dois retornos
de 50 minutos e último atendimento após um
mês. Porém, a partir das queixas e necessidade
do fortalecimento do vínculo terapêutico, houve
um total de sete atendimentos tornando-se um
Serviço de Plantão Psicológico estendido. A
paciente apresentava sintomas de depressão
grave com sintomas psicóticos, alucinações
auditivas, que se referiam à ideação suicida,
resultando em condições pouco estruturadas
de pensamento, atenção, memória, orientação
espaço-temporal e lateralidade. Entretanto,
demonstrou condições de estabelecer e de
manter o contrato terapêutico, estabelecendo
vínculo terapêutico com a estagiária, após
a escuta ativa e acolhimento. Em um breve
histórico de vida, relatou que houve um
acontecimento traumático em sua adolescência,
não esclarecido por ela inicialmente,
culminando em tentativas de suicídio. Nos
últimos atendimentos, relatou sobre o que havia
acontecido em sua adolescência e se repetido
posteriormente, tratando-se de violência
sexual. Autores apontam que pessoas que
tentam suicídio buscam a confiança e o vínculo
com o terapeuta. O tratamento realizado
de forma franca, clara e honesta facilitou a
comunicação sem interferências, promovendo
o estabelecimento da confiança.

Abstract:
The present study refers to
the report of a clinical case attended in the
Psychological Plant Service, in a schoolclinic, located in the interior of the state of
São Paulo, by a trainee of the 5th year of the
Psychology course, to a patient of 46 years old.
The objective is to emphasize the importance
of establishing the therapeutic link in the care
of a patient with suicidal ideation. The service
has an initial service, two 50-minute returns
and the last service after one month. However, based on the complaints and the
need to strengthen the therapeutic link, there were a total of seven visits becoming
an Extended Psychological Services. The patient presented symptoms of severe
depression with psychotic symptoms, auditory hallucinations, which referred to suicidal
ideation, resulting in unstructured conditions of thought, attention, memory, spatiotemporal orientation and laterality. However, it demonstrated conditions to establish
and maintain the therapeutic contract, establishing a therapeutic link with the trainee,
after active listening and reception. In a brief history of life, she reported that there
was a traumatic event in her teens, not clarified by her at first, culminating in suicide
attempts. In the last hearings, he reported on what had happened in his adolescence
and was repeated later, in the case of sexual violence. Authors point out that people
who attempt suicide seek trust and bonding with the therapist. The treatment performed
in a frank, clear and honest manner facilitated communication without interference,
promoting the establishment of trust.
