Abstract: Warfarin, a widely prescribed oral anticoagulant, is quite complicated in its management due to a narrow therapeutic window and dose-response variability requiring stringent follow-up. During the COVID-19 pandemic, telepharmaceutical monitoring presented itself as a feasible alternative in overcoming those restrictions barring personal attendance, ensuring patient safety and continuity of care. Such a method involves technologies like applications and drive-thru services which would allow remote monitoring and education of patients. The objective of this study was to review the impact of this remote pharmaceutical service on adherence to treatment with coumarin anticoagulants. Therefore, 12 studies were reviewed, published between 2019 and 2024, into databases like SciELO, PubMed, and LILACS, under specific inclusion criteria. The outcome results highlighted a better patient adherence to treatment and improvement in TTR among patients being kept in touch remotely, but efficiency and safety found at par with on-office visits. Thus, it leads to the conclusion that tele-monitoring is a very promising strategy to improve the management of anticoagulants, although technology and accessibility barriers still need to be addressed, specifically in the public system, as well as training of professionals.
