O SERVIÇO DE FARMÁCIA CLÍNICA COMO FERRAMENTA PARA IDENTIFICAR POSSÍVEIS INTERAÇÕES MEDICAMENTOSAS NAS PRESCRIÇÕES DA ATENÇÃO PRIMÁRIA, SECUNDÁRIA E TERCIÁRIA EM SAÚDE DE UM MUNICÍPIO DE PEQUENO PORTE DO OESTE DE SANTA CATARINA
Pacientes em politerapia são particularmente expostos a interações medicamentosas que podem levar a reações adversas a medicamentos ou falha terapêutica. Esta pesquisa teve como objetivo discutir o papel do farmacêutico clínico no cuidado hospitalar e no Sistema Único de Saúde (SUS) através da análise de prescrições médicas. Foi realizada um estudo retrospectivo durante o ano de 2019, em 2160 prontuários do ano de 2019, através de um protocolo que foram avaliados a presença de informações da posologia, potencial de interações medicamentosas, consistência entre fármaco prescrito e diagnóstico clínico e levantamento dos fármacos mais prescritos e as possíveis interações entre eles. Para a análise dos dados coletados utilizou-se de literatura científica e acadêmica apropriada, bem como o aplicativo MedSUS do Ministério da Saúde. As prescrições foram divididas em atenção primária, secundária e terciária em saúde. Foram encontradas interações entre medicamento/medicamento e medicamento/alimento. Também foram encontradas incompatibilidades em alguns diagnósticos, em outros, ausência de posologia e ainda alguns prontuários com prescrições ilegíveis. O significativo número de interações e erros encontrados nas prescrições médicas reforçam o papel e a importância que o farmacêutico clínico tem em desempenhar sua função em uma equipe multiprofissional nas unidades hospitalares, sobretudo revisando as prescrições médicas quando direcionados aos cuidados dos pacientes.

Abstract:
Polytherapy patients are particularly exposed to drug interactions that can lead to adverse drug reactions or therapeutic failure. This research aimed to discuss the role of the clinical pharmacist in hospital care and in the Unified Health System (SUS) through the analysis of medical prescriptions.
