Abstract: Syphilis during pregnancy remains a public health issue in Brazil, indicating deficiencies in prenatal care. Although diagnosis and treatment are effective, transmission to the baby still occurs. This literature review highlights that the main obstacle in Primary Health Care is the difficulty in treating the pregnant women's sexual partners, which maintains the circulation of the disease and the risk of reinfection. Other challenges include some professionals' reluctance to administer penicillin, deficiencies in the healthcare system, and social vulnerabilities that affect patients. To address the issue, a comprehensive approach is essential, including health education, professional training, and the implementation of policies that consider social and behavioral conditions that perpetuate the infection.
