Abstract: Hypertension and diabetes are two of the most prevalent and impactful health conditions worldwide, affecting millions of people. Both are considered chronic diseases that require constant management to prevent serious complications. In primary healthcare, a fundamental role is played in the prevention, early diagnosis, and treatment of these conditions, along with providing continuous support to patients. This article aims to address the nurse's role in the care of patients with Arterial Hypertension and Diabetes Mellitus. This present study is a literature review providing a comprehensive and updated view of the current state of knowledge on a specific topic, identifying gaps, conflicts, or trends in research. It can be argued that the nurse engages in assessment and monitoring activities, health education, and health promotion. It is concluded that the nurse, being present in practically every moment of patients' contact with the healthcare unit, establishes a continuous connection, which is essential for building a relationship of trust. This facilitates communication, understanding individual needs, and promoting treatment adherence.
