Abstract: The aim of this comprehensive review was to explore the mechanisms involved in metabolic bone changes around dental implants in diabetic patients, as well as the available therapies to improve osseointegration. A literature search was conducted in electronic databases PubMed, Cochrane Library, and embase to identify relevant studies. A significant body of evidence has been identified, demonstrating the negative impact of diabetes mellitus on bone health and impaired osseointegration. This can be attributed to several factors, including the overexpression of reactive oxygen species (ROS), accumulation of advanced glycation end products (AGEs), impaired angiogenesis and bone-related biomarkers. Various therapeutic approaches have been proposed to enhance dental implant osseointegration in diabetic patients. These include insulin therapy, hypoglycemic agents, parathyroid hormone therapy, implant surface modification, naturally occurring substances, mesenchymal stem cell management, gene expression and growth factor modulation. Despite the potential for numerous therapies, either for glycemic control or to directly assist with osseointegration, additional preclinical and clinical investigations are necessary to validate their effectiveness and safety.
