Abstract: The accelerated population aging and the prevalence of Chronic Non-Communicable Diseases (CNCDs) pose challenges to the public health system. This study, a narrative literature review, aims to analyze how Primary Health Care (PHC) can ensure comprehensive care for the elderly. The search was conducted in the LILACS, MEDLINE, BDENF, and SciELO databases, with publications between 2020 and 2025. The results show that, from the perspective of the elderly, care in the Family Health Strategy (FHS) remains centered on the biomedical model, focused on medical consultations and drug prescription, neglecting a holistic approach. Structural challenges are identified, such as network fragmentation, the impact of new funding policies, and uncoordinated care, resulting in avoidable hospitalizations. It is concluded that qualifying assistance requires strengthening PHC, valuing the multidisciplinary team, and reorienting the model towards care centered on the autonomy and functional capacity of the elderly person.
