Abstract: The concept of an Age-Friendly City, proposed by the World Health Organization (WHO), is based on the promotion of active aging through the optimization of opportunities related to health, participation, and security. This approach seeks to ensure quality of life and social inclusion for the elderly population in a context of accelerated demographic aging. Recent studies, particularly in Latin America and Brazil, highlight advances in the recognition of this paradigm but also reveal significant gaps in its implementation. The main challenges identified include the scarcity of municipal resources, the fragmentation of public policies, the insufficient effective participation of older adults in decision-making processes, and the absence of standardized metrics for evaluating results. Structural issues such as sociais inequality, architectural barriers, inadequate public transportation, and the lack of adapted health services hinder the consolidation of truly inclusive urban environments. Nevertheless, local initiatives demonstrate transformative potential when combined with participatory governance, technological innovation, and intersectoral policies. It is concluded that the construction of age-friendly cities requires greater integration of policies, continuous investment, and the strengthening of social participation in order to overcome inequalities and ensure dignified, healthy, and participatory aging.
