Abstract: Objective: To analyze the main nursing care aspects related to the administration, monitoring, and prevention of complications associated with Total Parenteral Nutrition (TPN). Methodology: Narrative review. Data collection took place in March 2026, using the Virtual Health Library, SciELO, and Google Scholar databases, using the descriptors "total parenteral nutrition," "nursing care," "nutritional therapy," and "patient safety." Articles available in full text, in Portuguese, English, and Spanish, were included. The data were analyzed descriptively and organized into thematic categories. Results: TPN is indicated for patients unable to use their gastrointestinal tract. Its main indications include conditions such as intestinal obstructions, paralytic ileus, severe pancreatitis, and critical states. The nursing role is fundamental, involving preparation and administration with aseptic technique, prescription verification, use of an infusion pump, and continuous monitoring. Complications can be infectious, metabolic, or mechanical, and are prevented through protocols, safety bundles, staff training, and continuous monitoring, ensuring greater safety and quality of care. Conclusion: It is concluded that the systematization of care, combined with the use of institutional protocols and continuing education, is crucial for the quality and safety of parenteral nutritional therapy.
