NOTIFICAÇÃO DE NEAR-MISS PARA PREVENÇÃO DE ERROS DE MEDICAÇÃO
Trata-se de um estudo transversal, com abordagem quantitativa que teve como objetivo identificar e analisar as notificações de near-miss relacionadas à cadeia medicamentosa, e sua importância na prevenção de erros de medicação, em uma organização hospitalar da cidade de Salvador, Bahia. A coleta de dados se deu através do levantamento retrospectivo das notificações de near-miss dos últimos doze meses. Constatou-se 1.777 notificações de incidentes, sendo 597 (34%) near-miss, destes, 521 (87%) estavam relacionados a erros de medicação. Observou-se uma cultura de notificação disseminada no hospital, com números representativos, sendo ainda um desafio o aumento das notificações de near-miss. Referente às etapas da cadeia medicamentosa, identificamos que o maior número de notificação de near-miss estava relacionado à etapa de prescrição de medicamento. A ocorrência de eventos adversos associado a erro de medicação no hospital se mostrou bastante inferior em relação ao near-miss, demonstrando o resultado da busca contínua de práticas mais seguras no uso de medicamento e maior segurança do paciente.

Abstract:
It is cross-sectional study with a quantitative approach aimed to identify and analyze the near-miss of notifications related to drug chain, and its importance in preventing medication errors in a hospital organization in the city of Salvador, Bahia. Data collection was made through the retrospective study of near-miss notifications for the last twelve months. It was found 1,777 reports of incidents, and 597 (34%) of near-mis, of these, 521 (87%) were related to medication errors. There was widespread notification culture in the hospital, with representative numbers to be a challenge increased near-miss notifications. Regarding the stages of the drug chain, we identified that the greatest near-miss reporting number was related to the stage of prescription medicine. The occurrence of adverse events associated with medication errors in the hospital proved quite lower than the near-miss, demonstrating the result of continuous search for safer practices in the use of edication and increased patient safety.
