Abstract: INTRODUCTION: Melanoma is an aggressive cutaneous neoplasm associated with mutations in genes that regulate the cell cycle. Among these, CDK4 stands out; its germline alteration, although rare, significantly increases the risk of the disease. CASE REPORT: The base case report for analysis was of a 56-year-old man with invasive cutaneous melanoma on his back, a strong family history (father and cousin), and a germline CDK4 mutation. RESULTS AND DISCUSSION: Digital platforms such as Invitae, QCancer, and Ask2Me indicated an elevated risk for melanoma and other cancers, with a marked discrepancy compared to the non-carrier population. Understanding risk factors and early detection strategies is crucial for effective prevention and treatment. In this context, the majority of patients with in situ or early-stage melanoma will be cured by primary excision alone. CONCLUSION: It falls to genetic counseling to assess the risk of transmitting the CDK4 gene mutation to future generations.
