INTRODUÇÃO: O melanoma é uma neoplasia cutânea agressiva associada a mutações em genes reguladores do ciclo celular. Entre elas, destaca-se o CDK4, cuja alteração germinativa, embora rara, aumenta expressivamente o risco da doença.  RELATO DE CASO: O relato do caso para análise foi de um homem, 56 anos, melanoma cutâneo invasivo em dorso, com forte agregação familiar (pai e prima) e mutação germinativa em CDK4.  RESULTADOS E DISCUSSÃO: Plataformas digitais como Invitae, QCancer e Ask2Me indicaram risco elevado de melanoma e outros cânceres, com discrepância acentuada em relação à população não portadora. Compreender os fatores de risco e as estratégias de detecção precoce é crucial para a prevenção e o tratamento eficaz. Nesse contexto, a maioria dos pacientes com melanoma in situ ou em estágio inicial será curada apenas com a excisão primária.  CONCLUSÃO: Cabe ao aconselhamento genético avaliar o risco de transmissão da mutação no gene CDK4 para as próximas gerações.

  • Abstract: INTRODUCTION: Melanoma is an aggressive cutaneous neoplasm associated with mutations in genes that regulate the cell cycle. Among these, CDK4 stands out; its germline alteration, although rare, significantly increases the risk of the disease. CASE REPORT: The base case report for analysis was of a 56-year-old man with invasive cutaneous melanoma on his back, a strong family history (father and cousin), and a germline CDK4 mutation.  RESULTS AND DISCUSSION: Digital platforms such as Invitae, QCancer, and Ask2Me indicated an elevated risk for melanoma and other cancers, with a marked discrepancy compared to the non-carrier population. Understanding risk factors and early detection strategies is crucial for effective prevention and treatment. In this context, the majority of patients with in situ or early-stage melanoma will be cured by primary excision alone.  CONCLUSION: It falls to genetic counseling to assess the risk of transmitting the CDK4 gene mutation to future generations.
