Abstract: Coronary Artery Disease (CAD) is one of the leading causes of cardiovascular morbidity and mortality, associated with atherosclerosis and genetic and behavioral factors. Recent evidence demonstrates that physical exercise (PE) plays a fundamental role in the management of CAD, in addition to pharmacological treatment. This review analyzed studies involving adults with CAD who underwent structured PE programs. The findings indicate that PE improves cardiorespiratory fitness, autonomic modulation, metabolic and inflammatory profiles, endothelial function, and quality of life, while also reducing overall cardiovascular risk. It is concluded that physical exercise is an effective and safe therapeutic strategy and should be systematically and individually incorporated into the treatment of CAD.
