Abstract: Warfarin is a drug used mainly as a way to prevent and treat thromboembolic events, administered orally and with good availability, whose treatment requires complex pharmacotherapeutic management, individualized doses and which are adjusted according to the RNI values of the patient. Objective: This work aims, mainly, to identify, by means of self-report, changes in the pharmacotherapy of chronic use of patients followed in two outpatient clinics of university hospitals in Belo Horizonte, as well as whether these drugs interact with warfarin and to analyze the influence of these interactions on the patients' TTR values. Method: Descriptive study, in which data were collected through telephone contact with patients from July to December 2018 (T0), April to July 2019 (T1) and August 2019 to January 2020 (T2), to identify possible changes in the pharmacotherapy in use, such as included, withdrawn, with increased or decreased dose, in addition to warfarin. Results and Discussion: Drug interactions in both outpatient clinics were considered, and the highest percentage was identified at HC-UFMG, and of these, interactions with warfarin were mostly classified as moderate. These interactions interfere with the control of the patients' RNI, and corroborate that pharmaceutical strategies are used to improve the quality of life and pharmacotherapy of the patients in question. Conclusion: It was not possible to correlate the influence of drug interactions on TTR, however a critical evaluation of patients' pharmacotherapy is essential, aiming in addition to health education practices and effective transitions between primary and secondary care.
