MELANOMA DE CORÓIDE EM PACIENTE OLHO ÚNICO: BRAQUITERAPIA X ENUCLEAÇÃO
O melanoma de coróide representa cerca de 70% dos tumores oculares malignos¹. Geralmente é unilateral e assintomático, quando apresenta sintoma, na maioria das vezes, é a baixa acuidade visual. As terapêuticas indicadas na maioria dos casos são a braquiterapia e a enucleação. Relatar caso de paciente olho único com diagnóstico de melanoma de coróide de dimensões elevadas, com sucesso terapêutico após braquiterapia. Relato de caso: paciente masculino, 66 anos, queixa baixa visão em olho esquerdo (OE) há 4 meses. Portador de Glaucoma neovascular avançado em olho direito (OD). Fundoscopia e Mapeamento de Retina do OE evidenciou Retinopatia Diabética Proliferativa e massa sub-retiniana temporal amarronzada, sugestiva de melanoma. ECO-B de OE mostrou massa cupuliforme em região temporal, com 11,5mm de altura e 9,8mm de base. Iniciada Braquiterapia com Iodo 125. ECO-B pós tratamento mostrou redução significativa no tamanho tumoral e melhora da visão. Diante de paciente olho único optou-se pela Braquiterapia, apesar de não ser a opção terapêutica indicada pelo tamanho do tumor. A redução tumoral foi significativa e a tentativa de preservar certa acuidade visual foi alcançada com sucesso. Conclusão: A conduta diante do melanoma de coróide considera, sobretudo, o tamanho do tumor, sendo que para tumores maiores de 10mm indica-se enucleação e para tumores menores a Braquiterapia. Entretanto, as condutas devem ser individualizadas e com critérios bem fundamentados buscando melhor resultado e qualidade de vida para cada paciente. Através deste capítulo buscou-se apresentar um caso de paciente olho único com diagnóstico de melanoma de coróide de dimensões elevadas e com sucesso terapêutico após braquiterapia, apesar de não ser indicação padrão dessa modalidade de tratamento diante do tamanho tumoral, baseando-se em uma conduta individualizada.

Abstract:
Choroidal melanoma represents about 70% of malignant eye tumors¹. It is usually unilateral and asymptomatic, when it presents symptoms, in most cases, it is low visual acuity. The therapies indicated in most cases are brachytherapy and enucleation. To report the case of a single eye patient diagnosed with large choroidal melanoma, with therapeutic success after brachytherapy. Case report: male patient, 66 years old, complaining of low vision in the left eye (LE) for 4 months. Patient with advanced neovascular glaucoma in the right eye (RE). Fundoscopy and Retinal Mapping of the LE showed Proliferative Diabetic Retinopathy and a brownish temporal subretinal mass, suggestive of melanoma. EO-B ECO showed a cupuliform mass in the temporal region, 11.5mm high and 9.8mm at the base. Iodine 125 brachytherapy was started. Post-treatment ECO-B showed a significant reduction in tumor size and improved vision. Faced with a single eye patient, Brachytherapy was chosen, despite not being the indicated therapeutic option due to the size of the tumor. Tumor reduction was significant and the attempt to preserve some visual acuity was successfully achieved. Conclusion: The management of choroidal melanoma takes into account, above all, the size of the tumor, and for tumors larger than 10mm, enucleation is indicated, and for smaller tumors, Brachytherapy is indicated. However, the conducts must be individualized and with well-founded criteria, seeking a better result and quality of life for each patient. Through this chapter, we sought to present a case of a single eye patient diagnosed with large choroidal melanoma and with therapeutic success after brachytherapy, although it is not a standard indication of this treatment modality given the tumor size, based on an approach individualized.
