Abstract: Depressive and mixed anxiety-depressive disorders are highly prevalent among women in Primary Care settings, especially in the Amazon region, where geographic isolation and sociocultural vulnerabilities intensify psychological distress. This report describes three years of clinical experience in a Fluvial Primary Care Unit (UBSF) located in rural Itacoatiara, Amazonas, with emphasis on the predominance of cases among women undergoing menopausal transition. Favorable therapeutic response was observed with a combined approach including non-pharmacological measures and selective serotonin reuptake inhibitors (SSRIs). Major challenges included stigma, difficulties ensuring follow-up due to population mobility, and limited access to specialized services. This experience reinforces the central role of Primary Care in the early identification, management, and longitudinal follow-up of mental disorders in Amazonian riverside communities.
