Abstract: Objective: To review current evidence on uterine fibroid management, comparing medical options—particularly oral gonadotropin-releasing hormone (GnRH) antagonists such as relugolix and linzagolix—with minimally invasive procedures like laparoscopic myomectomy and uterine artery embolization. Methods: A narrative literature review was conducted in PubMed, Embase, Cochrane Library, and Scielo, including articles published between 2020 and 2024. Randomized clinical trials, systematic reviews, meta-analyses, and official guidelines were selected. The PICO strategy defined women with symptomatic fibroids (P), treated with oral GnRH antagonists (I), compared with minimally invasive techniques (C), evaluating volume reduction, symptom control, and safety (O). Eight studies met inclusion criteria. Results: Oral GnRH antagonists showed significant efficacy in reducing fibroid volume and controlling bleeding, with favorable tolerability and safety during long-term use. Minimally invasive procedures remain relevant for uterine preservation and refractory cases. Conclusion: Oral GnRH antagonist therapy emerges as an effective, reversible, and less invasive alternative to surgical approaches, supporting individualized and evidence-based management of uterine fibroids.
