INTUSSUSCEPÇÃO INTESTINAL POR MELANOMA METASTÁTICO EM INTESTINO DELGADO - RELATO DE CASO
: Introdução: O melanoma é um câncer de pele pouco comum, representando 3% do total das neoplasias malignas de pele. Pode se comportar de forma agressiva com metástases a distância, inclusive no trato gastrointestinal (TGI), sendo neste caso a localização mais comum em intestino delgado. Objetivos e metodologia: O foco da pesquisa é apresentar e discutir um caso de melanoma metastático complicado abordado cirurgicamente. As informações foram obtidas pela equipe assistente durante o acompanhamento do caso da paciente e foi realizada revisão de literatura. Apresentação do caso: Paciente feminina, negra, 45 anos, com diagnóstico de melanoma metastático interna com quadro de obstrução intestinal e tomografia compatível com intussuscepção intestinal. Submetida a tratamento cirúrgico, foi identificada lesão tumoral intraluminal ocasionando intussuscepção intestinal. Realizada enterectomia segmentar com anastomose. Laudo anatomopatológico confirmou melanoma metastático. Comentários finais: Estabelecer no pré-operatório o diagnóstico de metástase em intestino delgado é difícil devido a inespecificidade de sintomas e limitações de métodos diagnósticos. O diagnóstico definitivo é obtido após exploração cirúrgica e ressecção da lesão. Com o presente relato salientamos que pacientes com melanoma com queixas gastrointestinais persistentes ou abdome agudo, devem ter como suspeita clínica metástase gastrointestinal, para que a propedêutica necessária seja estabelecida e o tratamento oncológico adequado seja realizado.

Abstract:
Introduction: Melanoma is an uncommon skin cancer, representing 3% of all malignant skin neoplasms. It may behave aggressively with distant metastasis, including the gastrointestinal tract (TGI), in which case, the most common location is at the small bowel. Objectives and methodology: The focus of the research is to present and discuss a complicated metastatic melanoma case surgically approached. The information was obtained by the assistant team during the follow-up of the patient's case and literature review was performed. Case report: Female patient, black, 45 years old, diagnosed with metastatic melanoma hospitalized with bowel obstruction and tomography compatible with intestinal intussusception. The patient underwent surgical treatment and an intraluminal jejunal tumor lesion was identified, causing jejuno-jejunal intussusception. Segmental enterectomy with anastomosis was performed. Anatomopathological report confirmed metastatic melanoma. Final comments: Establishing preoperative diagnosis of small bowel metastasis is difficult due to the non-specificity of symptoms and limitations of diagnostic methods. The definitive diagnosis is obtained after surgical exploration and resection of the lesion. With the present report we emphasize that melanoma patients with persistent gastrointestinal complaints or acute abdomen should have as a clinical suspicion gastrointestinal metastasis, so the necessary propaedeutics is established and the appropriate cancer treatment performed.
