INTERVENÇÕES FARMACÊUTICAS COMO INSTRUMENTO DE SEGURANÇA DO PACIENTE NA TERAPIA ANTINEOPLÁSICA NO HOSPITAL REGIONAL DO BAIXO AMAZONAS
Intervenção farmacêutica definese
como todas as atuações nas quais o
farmacêutico participa efetivamente na tomada
de decisões na terapêutica dos pacientes. No
ambiente hospitalar, a atenção farmacêutica
ocorre de maneira integrada com a equipe
multiprofissional, com ênfase na avaliação
da prescrição, monitoramento da terapia
medicamentosa e do quadro clínico do
paciente. A análise de prescrição médica pelo
farmacêutico promove uma farmacoterapia
segura e racional, uma vez que as intervenções
realizadas durante essa análise constituem
uma ferramenta eficaz na detecção e prevenção
de eventos adversos, contribuindo com a
qualidade assistencial. Objetivou-se analisar
as intervenções farmacêuticas em prescrições
médicas de pacientes em tratamento oncológico.
Estudo retrospectivo, quantitativo e documental
no período de Janeiro a Dezembro de 2017
em um hospital público no Oeste do Pará.
Levantamento de dados através de planilhas
de intervenções farmacêuticas em prescrições
médicas de terapia antineoplásica. Foram
realizadas 168 intervenções farmacêuticas: 61%
relacionadas à adequação de concentração
final, 23% a respeito de incompatibilidade de
diluentes, 4% sobre interação medicamentosa
e 4% acerca de incompatibilidade medicamento. As intervenções farmacêuticas mais
frequentes foram relacionadas à adequação de concentração final e incompatibilidade
de diluentes, com repercussão em redução de danos como: interferências na
biodisponibilidade do princípio ativo, falhas da efetividade terapêutica, administração
de superdosagens e desperdícios de medicamentos. Portanto, o estudo demonstrou
a relevância da atuação do farmacêutico clínico dentro da equipe multiprofissional,
contribuindo com a segurança do paciente mitigando riscos adversos evitáveis.

Abstract:
Pharmaceutical intervention is defined as all the actions in which the
pharmacist effectively participates in the decision making in the therapy of the patients.
In the hospital environment, pharmaceutical care occurs in an integrated manner with the
multiprofessional team, with emphasis on the evaluation of the prescription, monitoring
of the drug therapy and the clinical picture of the patient. The analysis of medical
prescription by the pharmacist promotes safe and rational pharmacotherapy, since the
interventions performed during this analysis are an effective tool in the detection and
prevention of adverse events, contributing to the quality of care. The objective was to
analyze the pharmaceutical interventions in medical prescriptions of patients undergoing
oncological treatment. Retrospective, quantitative and documentary study in the period
from January to December 2017 in a public hospital in the west of Pará. Data collection
through spreadsheets of pharmaceutical interventions in medical prescriptions of
antineoplastic therapy. There were 168 pharmaceutical interventions: 61% related to
final concentration suitability, 23% regarding diluent incompatibility, 4% drug interaction
and 4% drug incompatibility. The most frequent pharmaceutical interventions were
related to the suitability of final concentration and incompatibility of diluents, with
repercussion in harm reduction such as: interferences in the bioavailability of the active
principle, failures of therapeutic effectiveness, administration of drug overdoses and
waste. Therefore, the study demonstrated the relevance of the performance of the
clinical pharmacist within the multiprofessional team, contributing to patient safety by
mitigating avoidable adverse risks.
