INTERVENÇÃO CIRÚRGICA MEDIATA X IMEDIATA EM FRATURAS MANDIBULARES
A mandíbula, por ser um dos ossos mais proeminentes da face, é a estrutura óssea mais acometida por traumas maxilofaciais. Existem divergências quanto ao tempo aconselhado para intervenção cirúrgica nas fraturas mandibulares tendo em vista as possíveis complicações, conflitando características desse trauma quanto ao conceito de cirurgia emergencial ou eletiva. O objetivo do presente trabalho é realizar uma revisão de literatura para comparar as opiniões presentes quanto ao momento de escolha da intervenção cirúrgica. Foi realizada uma busca em sítios de dados eletrônicos PubMed, e Bireme utilizando os descritores “Mandibular Fracture”; Emergency”;” Complications”; utilizando como critérios de inclusão e exclusão: artigos publicados entre 2008-2019, escritos em língua inglesa e portuguesa e com resumos diretamente relacionados ao tema, sendo selecionados 10 artigos científicos publicados em periódicos. O resultado da análise dos artigos evidenciou grande diferença entre os padrões de atendimento entre os autores e ao país que foram publicados, variando desde protocolos emergenciais de pronto-atendimento até tratamento cirúrgico após 4 dias da injúria, além de não evidenciar diferença nas complicações ao paciente operado em períodos divergentes. Conclui-se que a incongruência entre os padrões de atendimento ao paciente com trauma mandibular, sem influência nas complicações ao paciente, torna a intervenção cirúrgica imediata uma terapêutica questionável.

Abstract:
The mandible, being one of the most prominent bones of the face, is the bone structure most affected by maxillofacial traumas. There are divergences regarding the recommended time for surgical intervention in mandibular fractures in view of the possible complications, conflicting characteristics of this trauma as to the concept of emergency or elective surgery. The objective of the present study is to perform a literature review to compare the present opinions regarding the moment of choice of the surgical intervention.
A search was performed on electronic data sites PubMed and Bireme using the descriptors "Mandibular Fracture"; Emergency ";" Complications "; using as inclusion and exclusion criteria: articles published between 2008-2019, written in English and Portuguese and with summaries directly related to the theme, being selected 10 scientific articles published in periodicals. The result of the analysis of the articles showed a great difference between the patterns of care between the authors and the country that were published, varying from emergency care protocols to surgical treatment after 4 days of injury, besides not showing differences in patient complications operated in divergent periods.
It is concluded that the incongruity between the patterns of care for the patient with mandibular trauma, without influence on the complications of the patient, makes immediate surgical intervention a questionable therapy. -
