Abstract: .INTRODUCTION AND OBJECTIVE: Polycystic ovary syndrome (PCOS) is the most common multifactorial endocrine disorder among women of reproductive age. The literature shows that changes in vitamin D homeostasis can interfere with proper follicular development. The objective of this study is to provide updated information on the influence of vitamin D deficiency on infertility in women with PCOS. METHODS: It is a narrative review based on the literature, with a qualitative approach, basic in nature and with a descriptive and explanatory objective. The search for articles to compose this literature review was carried out in the PubMed, Scopus, and Science Direct databases, using the descriptors ‘Polycystic Ovary Syndrome’; ‘Vitamin D’; ‘Fertility Woman’, with no limit established for the year of publication. Studies on vitamin D metabolism and its relationship with fertility, articles on vitamin D deficiency in women with PCOS, and studies investigating the effects of vitamin D supplementation on fertility parameters were included. Articles in which only the abstract was available, dissertations, and theses were excluded. A total of 198 articles were identified in the literature from the search using the aforementioned descriptors, and based on the eligibility criteria, 52 studies remained to support this review. RESULTS AND DISCUSSION: Vitamin D plays an important role in proper follicular development. The accumulation of adipose tissue in the abdominal region and low sun exposure, commonly observed in PCOS, contribute to vitamin D deficiency in this population. CONCLUSION: The data obtained in this review illustrate the influence of vitamin D on the development of infertility. However, the mechanisms involved in this relationship are not fully understood, and studies investigating the effectiveness of supplementation on infertility are scarce and controversial, demonstrating the need for further studies on this topic.
