Abstract: Abstract: Acute Ischemic Stroke (AIS) is caused by insufficient cerebral blood supply, accounting for 53-85% of stroke cases. Regarding pathophysiology, there are two mechanisms involved – thrombosis and embolism [2]. Artery of Percheron (AOP) infarction has a similar prevalence among men and women and risk factors include high cardiovascular risk and atheroembolic disease. PA is an unusual anatomical variation of the thalamo-perforating arteries, described as a vascular trunk that originates from P1 part of the posterior cerebral artery (PCA). [5]. Its function is to supply arterial blood to the bilateral paramedian regions of the thalamus. [4; 6] The classic triad of this abnormality includes hypersomnia, vertical ocular palsy and amnesic syndrome. [8] Diagnosis is clinical and complemented by imaging exams, which contribute to assessing the extent of ischemia and should be diagnosed earlier, aiming to limit the time of thrombolytic therapy. AOP infarction rarely occurs and it is clinically difficult to diagnose due to atypical acute neurological symptoms. We report a case of this 59 year-old female who had right-sided hemiparesis, aphasic and lowered level of consciousness, which was evaluated further and found to be the AOP infarct on CT brain and treated with antiplatelets.
