INDICAÇÕES E RESTRIÇÕES DA EPISIOTOMIA NO ATO CIRÚRGICO: AUSTERIDADE NA GARANTIA DO SUCESSO PROCEDIMENTAL COM A POLÊMICA DA VIOLÊNCIA OBSTÉTRICA
Introdução: Recentemente foi veiculado e debatido um movimento, de visibilidade crescente, que mira o combate da violência obstétrica e a adoção de práticas e protocolos mais humanistas na condução do parto. Com isso, nota-se uma ideia de vilanização impensada da efetuação da episiotomia, o que vai de encontro ao respeito do conhecimento teórico-prático do profissional médico e pode comprometer o sucesso do ato cirúrgico, logo, deve ser acompanhada de certa cautela. Metodologia: Revisão integrativa-analítica de obras publicadas nos últimos 2 anos, via PubMed, SciELO e MEDLINE, priorizando artigos de maior impacto e relevância para o tema. Resultados: Sabe-se que uma forte vertente na obstetrícia moderna preza pelo compartilhamento de decisões e pelo protagonismo materno. Com isso, surge um certo imbróglio quanto ao limite da autoridade dos personagens envolvidos no momento do parto, de modo que algumas preferências e anseios mostram-se, ocasionalmente, incompatíveis com a boa condução do procedimento e, consequente, com a preservação de um bom estado de saúde tanto da parturiente quanto do recém-nascido. Discussão: A condenação da episiotomia sem o conhecimento necessário para tal, mostra-se um óbice ao manejo de situações como distócia de ombro, parto pélvico e indícios de trauma perineal maior. Todavia, alerta-se que os índices brasileiros de realização desse procedimento superam consideravelmente o valor recomendado pela OMS. Conclusões: Deve haver equilíbrio e bom senso das partes presentes na tomada de decisões, de modo que a concepção seja conduzida da melhor maneira possível e que a prioridade comum seja o sucesso processual e a garantia da saúde dos envolvidos.

Abstract:
Introduction: Recently, a movement was launched and debated, with increasing visibility, which aims to combat obstetric violence and the adoption of more humanist practices and protocols in conducting childbirth. With this, there is an idea of unthinkable vilification of the performance of the episiotomy, which goes against the respect of the theoretical and practical knowledge of the medical professional and can compromise the success of the surgical act, therefore, it must be accompanied by some caution. Methodology: Integrative-analytical review of works published in the last 2 years, via PubMed, SciELO and MEDLINE, prioritizing articles of greater impact and relevance to the theme. Results: It is known that a strong strand in modern obstetrics values the sharing of decisions and the maternal role. With this, a certain imbroglio arises as to the limit of the authority of the characters involved at the time of delivery, so that some preferences and desires are occasionally incompatible with the proper conduct of the procedure and, consequently, with the preservation of health status of both the parturient and the newborn. Discussion: The condemnation of episiotomy without the necessary knowledge for this, proves to be an obstacle to the management of situations such as shoulder dystocia, pelvic delivery and evidence of major perineal trauma. However, it is warned that the Brazilian rates of carrying out this procedure considerably exceed the value recommended by the WHO. Conclusions: There must be balance and common sense of the parties present in decision-making, so that the conception is conducted in the best possible way and that the common priority is the procedural success and the guarantee of the health of those involved.
