IMPLEMENTAÇÃO DE BUNDLE DE PREVENÇÃO DE INFECÇÃO PRIMÁRIA DE CATETER VENOSO CENTRAL POR MEIO DA APRENDIZAGEM BASEADA EM EQUIPES
O Bundle, desenvolvido pelo Institute for Healthcare Improvement, consiste numa ferramenta estruturada, visando reduzir os índices de infecções relacionadas à assistência à saúde, por meio de um conjunto de medidas preventivas baseada em evidências científicas. Neste capítulo, objetiva-se delinear itinerário sobre a utilização de bundle de prevenção de infecção primária de corrente sanguínea por cateter venoso central (IPCS/CVC) em unidade de terapia intensiva adulto (UTIa), com base na experiência vivida por alguns dos autores. Portanto, refere-se à aplicação da aprendizagem baseada em equipe (ABE) como estratégia para efetivação do uso de bundle, com um grupo de profissionais de um hospital de médio porte, que presta atendimento de média e alta complexidade. Essa ferramenta é um impresso a ser preenchido pelo próprio profissional para cada paciente internado na UTIa. Deve ser checado como “conforme” (C) ou “não conforme” (NC), que congrega aspectos da segurança do paciente. Na fase inicial de implantação do bundle IPCS/CVC, identificou-se que 79% dos itens estavam preenchidos com “C”. Este resultado era incompatível com o índice da IPCS/CVC, uma vez que não houve a redução esperada em comparação com os dados anteriores. Assim, algumas ações gerenciais foram realizadas no setor pelo serviço do controle de infecção hospitalar para revisão dos processos de trabalho, por meio da ABE. A equipe responsável pela discussão e realinhamento das atividades incluídas nesse bundle destacou formas criativas para valorar a comunicação sobre os itens. Antes dessa intervenção (ABE), os profissionais desenvolviam as medidas de controle de infecção, mas o reforço colaborativo apontou que a intervisão, propiciou a construção de manejo facilitado, a autoaprendizagem e o registro no documento como um ato contínuo, ou seja, os próprios profissionais do setor mostraram possibilidades para a implementação do bundle IPCS/CVC. Concluindo, o conhecimento e apropriação de habilidades assertivas refletiram na redução da densidade de IPCS/CVC relacionada com assistência à saúde e taxa de adesão de 97% a esse bundle.

Abstract:
The Bundle, developed by the Institute for Healthcare Improvement, is a structured tool that aims to reduce the infection rate related to healthcare assistance through a set of preventive measures based on scientific evidence. In this chapter, the objective is to outline an itinerary on the use of a Bundle for the primary prevention bloodstream infection by central venous catheter (PIC/CVC) in an Adult Intensive Care Unit (ICU), based on the experience of some of the authors. Therefore, it refers to the set of a team-based learning (TBL) as a strategy to implement the use of the Bundle with a group of professionals from a medium-sized hospital, which provides medium and high complexity care. This tool is a form to be filled out by the professional for each patient admitted in the ICU. It has to be checked as "compliant" (C) or "not-compliant" (NC), which sets some patient safety's aspects. After the implementation process of the IPCS/CVC Bundle, it was identified that 79% of the items was filled with “C”. This result was incompatible with the IPCS/CVC rates once there was not the expected reduction, comparing the earlier data. Thus, it was given management actions in the sector has been done by the hospital infection control service aiming a working process review, using the TBL. The team responsible for the discussion and realignment of the activities included in this bundle highlighted creative ways to value communication about the items. Before this intervention (TBL), the professionals developed the infection control measures, but the collaborative reinforcement pointed out that the intervision provided the construction of facilitated management, self-learning and the registration in the document as a continuous act, i.e., the professionals of the sector showed possibilities for the implementation of the IPCS/CVC Bundle. In conclusion, the knowledge and appropriation of assertive skills reflected in the reduction of density of PICS/CVC related to health care and to 97% of adherence rate to this bundle.
